before  your  visit

At MesquiteEye, We make every effort to see that you are seen as quickly as possible and we understand that your time is valuable. When you call to make an appointment, we will ask for your personal information (name, address, insurance numbers, etc.) so that we can input all that into the computer before your visit. By doing so, you will spend less time filling out paperwork when you arrive.

In addition, it allows our staff to verify your insurance information prior to your visit and prevents the patient from any unexpected expenses. If you are not comfortable giving that information over the phone, we understand, but please arrive 30 minutes early to ensure you have time to read through and fill out any paperwork. (If we are unable to verify the patient's insurance, the patient will be responsible for any services performed at that visit, but we will provide the necessary paperwork so that the patient can file for reimbursement)

Things to bring:

• insurance information (including insurance card(s))
  
• any glasses or contacts that you have been wearing (unless we already have on file)
